Clinical Correlates of In-Hospital Mortality in Patients Undergoing Inferior Vena Cava Filter Placement for Acute Deep Vein Thrombosis

Background: It is reasonable to place an Inferior Vena Cava Filter (IVCF) when an acute deep vein thrombosis (DVT) of the lower limbs occurs in a patient with absolute contraindication to therapeutic anticoagulation. An additional potential reason for placing an IVCF is the need to stop therapeutic anticoagulation in a patient with acute DVT who must undergo urgent non-deferrable surgery. However, IVCFs are often used outside of such established indications and many authors argue about their actual utility, especially in terms of survival. In this retrospective study, we looked for clinical correlates of in-hospital mortality among patients who underwent IVCF placement, limiting our analysis to the cases for which a correct indication to IVCF placement existed. Methods: We retrospectively analyzed the electronic database of our University Hospital, searching for consecutive hospitalized patients who had acute DVT and underwent IVCF placement because of an established contraindication to therapeutic anticoagulation and/or because it was necessary to stop anticoagulation due to urgent surgery. The search covered the period between 1 January 2010 and 31 December 2020. Results: The search resulted in the identification of 168 individuals. An established contraindication to therapeutic anticoagulation was present in 116 patients (69.0%), while urgent non-deferrable surgery was the reason for IVCF placement in 52 patients (31.0%). A total of 24 patients (14.3%) died during the same hospital stay in which the IVCF was placed. Mortality rate was significantly higher in patients with a contraindication to anticoagulation than in patients who underwent IVCF placement because of urgent surgery (19.0% vs. 3.8%, OD 5.85 vs. 0.17). In-hospital mortality was also significantly higher among patients with chronic kidney disease and those who needed blood cell transfusion during hospitalization. Conclusions: This study provides novel information on clinical correlates of in-hospital mortality among patients with acute DVT who undergo IVCF. Prospective observational studies are needed to substantiate these findings.

The relationship between red cell distribution width, serum calcium ratio, and in-hospital mortality among patients with acute respiratory failure: A retrospective cohort study of the MIMIC-IV database.

To investigate the impact of RDW/CA (the ratio of red cell distribution width to calcium) on in-hospital mortality in patients with acute respiratory failure (ARF). This retrospective cohort study analyzed the data of 6981 ARF patients from the Medical Information Mart for Intensive Care (MIMIC-IV) database 2.0. Critically ill participants between 2008 and 2019 at the Beth Israel Deaconess Medical Center in Boston. The primary outcome of interest was in-hospital mortality. A Cox proportional hazards regression model was used to determine whether the RDW/CA ratio independently correlated with in-hospital mortality. The Kaplan-Meier method was used to plot the survival curves of the RDW/CA. Subgroup analyses were performed to measure the mortality across various subgroups. After adjusting for potential covariates, we found that a higher RDW/CA was associated with an increased risk of in-hospital mortality (HR = 1.17, 95% CI: 1.01-1.35, P = .0365) in ARF patients. A nonlinear relationship was observed between RDW/CA and in-hospital mortality, with an inflection point of 1.97. When RDW/CA ≥ 1.97 was positively correlated with in-hospital mortality in patients with ARF (HR = 1.554, 95% CI: 1.183-2.042, P = .0015). The Kaplan-Meier curve indicated the higher survival rates for RDW/CA < 1.97 and the lower for RDW/CA ≥ 1.97 after adjustment for age, gender, body mass index, and ethnicity. RDW/CA is an independent predictor of in-hospital mortality in patients with ARF. Furthermore, a nonlinear relationship was observed between RDW/CA and in-hospital mortality in patients with ARF.

Warfarin, not direct oral anticoagulants nor antiplatelet therapy, is associated with increased bleeding risk in emergency general surgery patients: implications in this new era of novel anticoagulants: An EAST Multicenter study.

To assess perioperative bleeding complications & in-hospital mortality in patients requiring emergency general surgery (EGS) presenting with a history of antiplatelet (AP) vs. direct oral anticoagulant (DOAC) vs warfarin use. Prospective observational study across 21 centers between 2019-2022. Inclusion criteria were age ≥ 18 years, & DOAC, warfarin or AP use within 24 hours of an EGSP. Outcomes included perioperative bleeding and in-hospital mortality. The study was conducted using ANOVA, Chi-square, and multivariable regression models. Of the 413 patients, 221 (53.5%) reported AP use, 152 (36.8%) DOAC use, & 40 (9.7%) warfarin use. Most common indications for surgery were obstruction (23% (AP), 45% (DOAC), 28% (warfarin)), intestinal ischemia (13%, 17%, 23%), & diverticulitis/peptic ulcers (7%, 7%, 15%). Compared to DOAC use, warfarin use was associated with significantly higher perioperative bleeding complication (OR 4.4 [2.0, 9.9]). There was no significant difference in perioperative bleeding complication between DOAC & AP use (OR 0.7 [0.4, 1.1]). Compared to DOAC use, there was no significant difference in mortality between warfarin use (0.7 [0.2, 2.5]) or AP use (OR 0.5 [0.2, 1.2]). After adjusting for confounders, warfarin use (OR 6.3 [2.8, 13.9]), medical history and operative indication were associated with an increase in perioperative bleeding complications. However, warfarin was not independently associated with risk of mortality (OR 1.3 [0.39, 4.7]), whereas intraoperative vasopressor use (OR 4.7 [1.7, 12.8)), medical history & postoperative bleeding (OR 5.5 [2.4, 12.8]) were. Despite ongoing concerns about the increase in DOAC use & lack of readily available reversal agents, this study suggests that warfarin, rather than DOACs, is associated with higher perioperative bleeding complications. However, that risk does not result in an increase in mortality, suggesting that perioperative decisions should be dictated by patient disease & comorbidities rather than type of antiplatelet or anticoagulant use.

Comparative analysis of the prognostic significance of the CURB-65 and CRB-65 scores and their modifications in hospital mortality assessment in patients with community-acquired pneumonia

Background. Mortality associated with community-acquired pneumonia (CAP) continues to be a crucial health problem worldwide. Correct assessment of CAP severity and the level of care is pivotal in the disease outcome.Aim. To evaluate the prognostic value of the CURB-65 and CRB-65 scores and their modifications in determining the risk of in-hospital mortality in patients with CAP.Materials and methods. The retrospective study included 1,412 patients with CAP aged over 18 years. In a population of 1,020 patients, which was subsequently split into test (n = 676) and training (n = 344) samples in the ratio 2 : 1, we compared the predictive value of the CURB-65 (confusion, urea &gt; 7 mmol / l, respiratory rate ≥ 30 / min, low blood pressure (BP), and age ≥ 65 years) and CRB-65 (confusion, respiratory rate ≥ 30 / min, low blood pressure (BP), and age ≥ 65 years) scores in identifying patients at high risk of in-hospital death. The specified scoring systems were modified by changing the cut-offs for each criterion to increase their accuracy. For comparison, we used the ROC analysis with the calculation of the area under the curve (AUC).Results. The modified CURB-65 score with new cut-off values (age &gt; 72 years, respiratory rate &gt; 21 / min, urea level &gt; 9.5 mmol / l, systolic blood pressure ≤ 105 mm Hg, and diastolic blood pressure ≤ 65 mm Hg) was more accurate than the original one in predicting death and was named CURB-72. The AUC for CURB-72 and CURB-65 was 0.946 (95% confidence interval (CI): 0.916–0.967) and 0.905 (95% CI: 0.869–0.934), respectively (p = 0.0034). The modified CRB-65 (CRB-72) score also outperformed the original model, but showed no statistically significant difference. While comparing the modified scoring systems, the new CURB-72 score surpassed the CRB-72 score and demonstrated maximum accuracy in identifying CAP patients at risk of in-hospital mortality (p = 0.0347).Conclusion. The modified CURB-65 (CURB-72) and CRB-65 (CRB-72) scores demonstrated potential for assessing the prognosis of CAP and are superior to classical scoring systems. CURB-72 showed the highest sensitivity and specificity.

Persistent Inflammation, Immunosuppression, and Catabolism Syndrome in Sepsis

Sepsis is a life-threatening organ dysfunction resulting from a dysregulated host response to infection. Early recognition and appropriate management improve in-hospital mortality in patients with sepsis. However, many patients progress to chronic critical illness (CCI). Persistent inflammation, immunosuppression, and catabolic syndrome (PICS), a new phenotype of CCI, is characterized by chronic low-grade inflammation, host immunosuppression, and catabolic-dominant weight loss. Due to the complexity of the pathophysiologic mechanisms of PICS, there is no effective treatment available. The aim of this review is to increase understanding of the pathophysiological mechanisms of PICS in sepsis, to summarize the diagnostic criteria and potential treatments for PICS, and to help clinicians adopt more comprehensive measures to improve the long-term prognosis of patients with PICS.

A new nomogram to predict in-hospital mortality in patients with acute decompensated chronic heart failure and diabetes after 48 Hours of Intensive Care Unit

BackgroundThe study set out to develop an accurate and clinically valuable prognostic nomogram to assess the risk of in-hospital death in patients with acute decompensated chronic heart failure (ADCHF) and diabetes.MethodsWe extracted clinical data of patients diagnosed with ADCHF and diabetes from the Medical Information Mart for Intensive Care III database. Risk variables were selected utilizing least absolute shrinkage and selection operator regression analysis, and were included in multivariate logistic regression and presented in nomogram. bootstrap was used for internal validation. The discriminative power and predictive accuracy of the nomogram were estimated using the area under the receiver operating characteristic curve (AUC), calibration curve and decision curve analysis (DCA).ResultsAmong 867 patients with ADCHF and diabetes, In-hospital death occurred in 81 (9.3%) patients. Age, heart rate, systolic blood pressure, red blood cell distribution width, shock, β-blockers, angiotensin converting enzyme inhibitors or angiotensin receptor blockers, assisted ventilation, and blood urea nitrogen were brought into the nomogram model. The calibration curves suggested that the nomogram was well calibrated. The AUC of the nomogram was 0.873 (95% CI: 0.834–0.911), which was higher that of the Simplified Acute Physiology Score II [0.761 (95% CI: 0.711–0.810)] and sequential organ failure assessment score [0.699 (95% CI: 0.642–0.756)], and Guidelines-Heart Failure score [0.782 (95% CI: 0.731–0.835)], indicating that the nomogram had better ability to predict in-hospital mortality. In addition, the internally validated C-index was 0.857 (95% CI: 0.825–0.891), which again verified the validity of this model.ConclusionsThis study constructed a simple and accurate nomogram for predicting in-hospital mortality in patients with ADCHF and diabetes, especially in those who admitted to the intensive care unit for more than 48 hours, which contributed clinicians to assess the risk and individualize the treatment of patients, thereby reducing in-hospital mortality.

Clinical characteristics and antimicrobial therapy of healthcare-associated carbapenem-non-susceptible gram-negative bacterial meningitis: a 16-year retrospective cohort study

ObjectiveHealthcare-associated Gram-negative bacterial meningitis is a substantial clinical issue with poor outcomes, especially for neurosurgical patients. Here, we aimed to study the characteristics and treatment options of patients with healthcare-associated carbapenem-non-susceptible (Carba-NS) Gram-negative bacterial meningitis.MethodsThis observational cohort study was conducted at a teaching hospital from 2004 to 2019. The clinical characteristics of patients with meningitis with Carba-NS and carbapenem-susceptible (Carba-S) bacilli were compared, and the antimicrobial chemotherapy regimens and outcomes for Carba-NS Gram-negative bacterial meningitis were analyzed.ResultsA total of 505 patients were included, of whom 83.8% were post-neurosurgical patients. The most common isolates were Acinetobacter spp. and Klebsiella spp., which had meropenem-resistance rates of 50.6% and 42.5%, respectively, and showed a markedly growing carbapenem-resistance trend. Kaplan–Meier curve analysis revealed that Carba-NS Gram-negative bacilli were associated with a significantly higher in-hospital mortality rate (18.8%, 35/186) compared to the Carba-S group (7.4%, 9/122; P = 0.001). For Carba-NS Enterobacterales meningitis, aminoglycoside-based and trimethoprim-sulfamethoxazole-based regimens yielded significantly higher clinical efficacy rates than non-aminoglycoside-based and non-trimethoprim-sulfamethoxazole-based regimens (69.0% vs. 38.7%, P = 0.019 and 81.8% vs. 46.9%, P = 0.036, respectively). For Carba-NS A. baumannii complex meningitis, tetracycline-based (including doxycycline, minocycline, or tigecycline) therapy achieved a significantly higher clinical efficacy rate (62.9%, 22/35) than the non-tetracycline-based therapy group (40.4%, 19/47; P = 0.044).ConclusionsOur findings revealed that Carba-NS Gram-negative bacilli are associated with higher in-hospital mortality in patients with healthcare-associated meningitis. The combination therapies involving particular old antibiotics may improve patients’ outcome.Trial registrationThis study was registered on the Chinese Clinical Trial Register under ChiCTR2000036572 (08/2020).

Which one predicts mortality better in acute ischemic stroke: negative vs positive acute-phase reactants

ABSTRACT Aim The aim of the study is to investigate the relationship between negative acute-phase reactants and positive acute-phase reactants with in-hospital mortality in patients diagnosed with acute ischemic stroke (AIS) in the emergency department (ED). Methods and Materials Patients aged 18 and older who presented to the ED of a tertiary hospital with AIS were included in the study. Demographic and clinical characteristics, laboratory parameters, acute-phase reactants, National Institutes of Health Stroke Scale (NIHSS), and outcome data of the included patients were recorded on a standard data form. Results A total of 588 patients were included in the study. When the in-hospital mortality of patients was examined, the mortality rate was 17.7%. In the analysis for predicting mortality, it was determined that albumin had the highest predictive power between the area under the curve (AUC) and the determined predictive values (AUC: 0.759, 95% CI 0.707–0.810, p < 0.001). The analyses of the study data revealed that albumin (<0.001) and TF (p = 0.049), which are negative acute-phase reactants, were independent predictors of mortality. According to our study data, in patients with AIS, for each unit decrease in albumin level at the time of ED admission, the risk of mortality increased by 0.868 times, and for each unit decrease in TF level, the risk of mortality increased by 0.593 times. Conclusion According to the study data, albumin and TF levels, which are negative acute-phase reactants, are independent determinants of in-hospital mortality in patients with acute ischemic stroke in the emergency department.

251 A Comparison of Neurosurgical Intervention and In-hospital Mortality in Patients With Intracranial Gunshot Wounds From Long Guns Versus Handguns: A Propensity-matched National Database Analysis

INTRODUCTION: Intracranial gunshot wounds (IC-GSW) are an understudied cause of mortality with indications for neurosurgical intervention. Historically, handguns have accounted for the majority of IC-GSW, but incidents involving long guns (rifles/shotguns) are rising. A comparison of neurosurgical intervention and in-hospital mortality by firearm type has yet to be fully explored. METHODS: The National Trauma Data Bank was queried from 2017-2019 for patients with IC-GSW presenting to a hospital with signs of life. Propensity score matching was performed between LG and HG groups based on demographics, comorbidities, Glasgow Coma Scale (GCS), and insurance type. Group differences were compared using paired Student’s t-tests and Pearson’s chi-square tests. A logistic regression model was developed to identify predictors of mortality. RESULTS: A total of 6,585 patients met inclusion criteria. Following one-to-one propensity score matching, 510 patients were identified and compared in each group. Seventy percent of patients with IC-GSW arrived with a GCS &lt; 8 (68.43% LG vs. 72.16% HG, p=0.19). Rates of neurosurgical intervention were similar between the LG and HG groups (20.49% vs. 23.53%, p = 0.26), however LG patients had a lower rate of in-hospital mortality (34.51% vs. 42.55%, p &lt; 0.01). Neurosurgical wound debridement (OR = 0.46, p&lt;0.001), foreign body removal (OR = 0.44, p &lt; 0.001), craniectomy (OR = 0.48, p &lt; 0.001), and ventriculostomy (OR = 0.58, p &lt; 0.01) were associated with lower odds of mortality. CONCLUSIONS: Patients with IC-GSW often present to the hospital in critical neurologic condition. Neurosurgical intervention is associated with lower odds of in-hospital mortality. Though both groups undergo neurosurgery at similar rates, patients with LG IC-GSW have a lower mortality rate. This study identifies a subset of patients who may particularly benefit from neurosurgery. Further research is needed to quantify the rationale behind these findings.

THE ROLE OF NEUTROPHILS TO LYMPHOCYTES RATIO AS PREDICTIVE BIOMARKER IN ACUTE MYOCARDIAL INFARCTION PATIENTS AT AL-GAMHURIA MODERN GENERAL HOSPITAL – ADEN-YEMEN

Inflammation plays a fundamental role in the development of atherosclerosis and in the pathogenesis of acute myocardial infarction (AMI). Neutrophils to lymphocytes ratio (NLR) was recognized as important inflammatory marker in predicting the severity and prognosis of AMI. The aim of this study is to determine the role of NLR as predictive biomarker in AMI patients at Al-Gamhuria Modern General Hospital. This study was considered a descriptive cross-sectional study, which was conducted in Al-Gamhuria Modern General Hospital from March through August 2022. The study includes 124 patients diagnosed as AMI and classified into two groups: 70 patients with low NLR and 54 patients with high NLR. Out of 124 patients, 56.5% of patients had low NLR and 43.5% had high NLR. Patients with high NLR were older (62.31 ± 7.30 vs. 57.80 ± 5.17 years), had higher prevalence of D.M., hyperlipidemia, more smokers and Khat chewers compared to patients with low NLR. High NLR group had more elevated CK-MB and Troponin T, more reduced ejection fraction (EF) and fractional shortening (FS) compared to low NLR group (P &lt; 0.001). High NLR group also had poorer Killip class (P &lt; 0.001), and were more prone to death inside the hospital compared to low NLR group (11.1% vs. 1.4%), (P= 0.042). As conclusion, NLR is a strong predictor of myocardial damage, myocardial dysfunction as well as in-hospital mortality in patients with AMI. NLR as an inflammatory marker can also predict the consequences on the heart post-MI.

U-Shaped Association between Serum Chloride Levels and In-Hospital Mortality in Patients with Congestive Heart Failure in Intensive Care Units.

Serum chloride level has clinical significance in the prognosis of heart failure. Little is known regarding the association between serum chloride levels and in-hospital mortality in patients with heart failure.This retrospective study used clinical data obtained from the Medical Information Mart for Intensive Care Database. The study cohort comprised patients who were categorized on the basis of their serum chloride levels, and the primary endpoint was in-hospital mortality. To assess the impact of serum chloride levels at the time of intensive care unit admission on in-hospital mortality, we used various statistical approaches, including multivariable logistic regression models, a generalized additive model, and a two-piecewise linear regression model. In addition, subgroup analysis was conducted to examine the robustness of the main findings.This study comprised 15,983 participants. When compared with the reference group (Q5), the groups with the highest (Q7) and lowest (Q1) blood chloride levels exhibited increased in-hospital mortality, with fully adjusted odds ratios (ORs) of 1.36 [95% confidence interval (CI): 1.08-1.71] and 1.25 (95% CI: 1-1.56), respectively. A U-shaped relationship was observed between blood chloride levels and in-hospital mortality, with the lowest risk observed at a threshold of 105.017 mmol/L. The effect sizes and corresponding CIs below and above the threshold were 0.969 (95% CI: 0.957-0.982) and 1.039 (95% CI: 1.002-1.076), respectively. Stratified analyses demonstrated the robustness of this correlation.The relationship between serum chloride levels and in-hospital mortality in patients with heart failure was U-shaped, with an inflection point of 105.017 mmol/L.

Comparison of In-Hospital Outcomes between Heart Failure Patients with Reduced and Preserved Ejection Fraction: A Single-Center Cross-Sectional Study

Objectives: Heart failure (HF) poses a significant global health burden, characterized by inadequate cardiac output and systemic organ dysfunction. This study aimed to compare in-hospital outcomes between patients with reduced (HFrEF) and preserved (HFpEF) ejection fraction presenting with congestive heart failure. Methodology: A cross-sectional, prospective study was conducted at the Department of Cardiology, National Institute of Cardiovascular Diseases, Karachi, Pakistan, from July 2022 to January 2023. Patients aged 35 to 80 years with congestive heart failure were included. Ejection fraction status, demographic data, and clinical parameters were assessed, with in-hospital mortality as the primary outcome. Results: Among 196 patients, 91 (46.4%) had HFrEF, and 105 (53.6%) had HFpEF. In-hospital mortality occurred in 23 (11.7%) patients. Mortality rates were significantly higher in HFrEF patients compared to HFpEF patients (17.6% vs. 6.7%, p=0.018). Age (&gt;60 years) and diabetes mellitus were significantly associated with in-hospital mortality (p=0.001 and p=0.036, respectively). Conclusion: This study highlights significantly higher in-hospital mortality rates in patients with reduced ejection fraction compared to preserved ejection fraction, underscoring the importance of considering ejection fraction status in assessing prognosis and guiding management strategies for patients with congestive heart failure.

Clinical Features and Outcomes of Shoshin Beriberi.

Shoshin beriberi is a fulminant form of wet beriberi, but there are no large-scale studies detailing the clinical features of this disease. We investigated the clinical features and outcomes of Shoshin beriberi using data from a nationwide database in Japan.Using the Diagnosis Procedure Combination database, we identified patients with Shoshin beriberi between July 2010 and March 2021. We retrospectively investigated the characteristics, comorbidities, treatment, and in-hospital mortality of patients with Shoshin beriberi. The chi-square test or Fisher's exact test was used for categorical variables, and the Mann-Whitney U-test was used for continuous variables.We identified 62 patients with Shoshin beriberi. The median (interquartile range) age was 63 (48-69) years. Furthermore, 54 patients were male (87%). The most common comorbidity was alcohol-related disorder (34%). The median (interquartile range) length of hospital and intensive care unit stays were 17 (range, 10-35) and 5 (range, 1-9) days, respectively. The proportion of patients who received venoarterial extracorporeal membrane oxygenation, intra-aortic balloon pump, continuous renal replacement therapy, and mechanical ventilation was 11, 5, 29, and 63%, respectively. Among the patients with Shoshin beriberi, 53% received 2 or more catecholamines or inotropes. The in-hospital mortality was 23%. Impaired consciousness at admission was significantly related to in-hospital death (P < 0.001).The present study is the first and largest to describe the clinical features of patients with Shoshin beriberi using a nationwide database. Impaired consciousness at admission was significantly associated with in-hospital death.

Clinical Efficacy of Beta-1 Selective Beta-Blockers Versus Propranolol in Patients With Thyroid Storm: A Retrospective Cohort Study.

Thyroid storm is the most severe manifestation of thyrotoxicosis. Beta-blockers are among the standard treatment regimens for this condition, with propranolol being the historically preferred option. However, 2016 guidelines issued by the Japan Thyroid Association and the Japan Endocrine Society recommend the use of beta-1 selective beta-blockers over nonselective beta-blockers, such as propranolol. Nevertheless, evidence supporting this recommendation is limited. Herein, we aimed to investigate the in-hospital mortality of patients with thyroid storms based on the choice of beta-blockers. Retrospective cohort study. The Diagnosis Procedure Combination database, a national inpatient database in Japan. Patients hospitalized with thyroid storm between April 2010 and March 2022. Propensity-score overlap weighting was performed to compare in-hospital mortality between patients who received beta-1 selective beta-blockers and those who received propranolol. Subgroup analysis was also conducted, considering the presence or absence of acute heart failure. Among the 2462 eligible patients, 1452 received beta-1 selective beta-blockers and 1010 received propranolol. The crude in-hospital mortality rates were 9.3% for the beta-1 selective beta-blocker group and 6.2% for the propranolol group. After adjusting for baseline variables, the use of beta-1 selective beta-blockers was not associated with lower in-hospital mortality (6.3% vs. 7.4%; odds ratio, 0.85; 95% CI, 0.57-1.26). Furthermore, no significant difference in in-hospital mortality was observed in patients with acute heart failure. In patients with thyroid storm, the choice between beta-1 selective beta-blockers and propranolol did not affect in-hospital mortality, regardless of the presence of acute heart failure. Therefore, both beta-1 selective beta-blockers and propranolol can be regarded as viable treatment options for beta-blocker therapy in cases of thyroid storm, contingent upon the clinical context.

Prognostic accuracy of early warning scores for predicting serious illness and in-hospital mortality in patients with COVID-19.

A simple bedside triage tool is essential to stratify COVID-19 patients in the emergency department (ED). This study aimed to identify an early warning score (EWS) that could best predict the clinical outcomes in COVID-19 patients. Data were obtained from medical records of 219 laboratory-confirmed COVID-19 positive patients. We calculated 13 EWSs based on the admission characteristics of the patients. Receiver operating characteristic (ROC) curve analysis was used to assess the performance of the scores in predicting serious illness and in-hospital mortality. The median patient age was 51 (38, 60) years, and 25 (11.4%) patients died. Among patients admitted with mild to moderate illness (n = 175), 61 (34.9%) developed serious illness. Modified National Early Warning Score (m-NEWS) (AUROC 0.766; 95% CI: 0.693, 0.839) and Rapid Emergency Medicine Score (REMS) (AUROC 0.890; 95% CI: 0.818, 0.962) demonstrated the highest AUROC point estimates in predicting serious illness and in-hospital mortality, respectively. Both m-NEWS and REMS demonstrated good accuracy in predicting both the outcomes. However, no significant difference was found between m-NEWS (p = 0.983) and REMS (p = 0.428) as well as some other EWSs regarding the AUROCs in predicting serious illness and in-hospital mortality. We propose m-NEWS could be used as a triage score to identify COVID-19 patients at risk of disease progression and death especially in resource-poor settings because it has been explicitly developed for risk stratification of COVID-19 patients in some countries like China and Italy. However, this tool needs to be validated by further large-scale prospective studies.

Factors associated with in-hospital mortality and morbidity of patients with dissection of aorta type A, undergoing repair surgery

Aim: Acute aortic dissection type A (AADA) is a life-threatening emergency with high mortality rates. Surgical intervention is the preferred and potentially life-saving treatment, but perioperative mortality remains a significant concern. Identifying the risk factors associated with surgical mortality, morbidity, and survival rates is essential. Therefore, this study aimed to characterize the preoperative and intraoperative risk factors for death during surgical repair of AADA. Material and Methods: We included 64 consecutive patients who underwent surgical repair between April 2013 and March 2021 in a tertiary heart center. Perioperative characteristics and variables were collected and analyzed to find any correlation between them and in-hospital Outcomes. Results: Aortic dissection surgery was performed on an average of 8 patients annually, accounting for approximately 1.7% (64/3757) of all open-heart surgery cases. The in-hospital mortality rate was 28% (18/64). The main predisposing factors for in-hospital death were cardiac failure (11.1%), stroke (16.7%), uncontrollable hemorrhage (27.8%), renal failure (11.1%), respiratory failure (11.1%), and postoperative multi-organ failure (22.2%). We found no associations between cardiopulmonary bypass duration and mortality. During the routine use of cerebral protective perfusion, we did not encounter any cases of coma. Left ventricular hypertrophy, a short distance of intimal tear to the aortic valve, and renal failure were associated with postoperative bleeding tendency. Conclusion: Our study showed that multiple factors such as heart failure, uncontrolled bleeding, and respiratory failure increase in-hospital mortality of patients with dissection of aorta type A, but duration of surgery does not have any association with in-hospital death.

Usefulness of lactate to albumin ratio for predicting in-hospital mortality in atrial fibrillation patients admitted to the intensive care unit: a retrospective analysis from MIMIC-IV database

BackgroundHigh lactate to albumin ratio (LAR) has been reported to be associated to with poor prognosis in patients admitted to the intensive care unit (ICU). However, its role in predicting in-hospital mortality in AF patients admitted to ICU has not been explored.MethodsThe Medical Information Mart for Intensive Care-IV (MIMIC-IV) database was used to retrieve information on patients who had been diagnosed with AF. X-tile software was utilized to determine the optimal cut-off LAR. Area under the receiver operating characteristic curves (AUC), calibration plots, and decision curve analysis (DCA) were conducted to assess the prediction performance of LAR for in-hospital mortality.ResultsFinally, 8,287 AF patients were included and 1,543 death (18.6%) occurred. The optimal cut-off value of LAR is 0.5. Patients in lower LAR (< 0.5) group showed a better in-hospital survival compared to patients in higher LAR (≥ 0.5) group (HR: 2.67, 95%CI:2.39–2.97, P < 0.001). A nomogram for in-hospital mortality in patients with AF was constructed based on multivariate Cox analysis including age, CCI, β blockers usage, APSIII, hemoglobin and LAR. This nomogram exhibited excellent discrimination and calibration abilities in predicting in-hospital mortality for critically ill AF patients.ConclusionLAR, as a readily available biomarker, can predict in-hospital mortality in AF patients admitted to the ICU. The nomogram that combined LAR with other relevant variables performed exceptionally well in terms of predicting in-hospital mortality.

Impact of Medicaid Expansion on Trauma Patients at Extreme Risk of Mortality: A Time-Series Analysis.

The 2014 expansion of Medicaid under the Affordable Care Act (ACA) reshaped healthcare delivery in the United States. This study assessed how Medicaid expansion affected in-hospital mortality in patients with extreme risk of mortality (EROM) from traumatic injuries. Data from inpatients aged 18-64 years, registered in the National Inpatient Sample between 2007 and 2020, and identified with trauma-related All-Patient Refined Diagnosis Related Groups (APRDRG) codes, were analyzed. Within this group, a subset of patients was selected based on the APRDRG classification identifying them as at EROM for the principal unit of analysis. The cohort was divided into high-implementation (HIR) and low-implementation (LIR) regions based on Medicaid expansion coverage. In-hospital mortality was assessed using interrupted time-series analysis. Sensitivity analyses considered seasonality, autocorrelation, and exogenous events. Analysis encompassed 70381 trauma inpatient stays, corresponding to 346659 patients based on National Inpatient Sample weighting. There was a consistent monthly decline in in-hospital mortality of .08% (95% CI: -.103 to -.048; P < .001) prior to Medicaid expansion, a trend unaffected by expansion. This pattern persisted across both LIR and HIR Medicaid implementation regions. Although Medicaid enrollment increased in HIR, that in LIR remained unchanged. Over the study period, the in-hospital mortality among severely injured patients consistently decreased, and this trend was not influenced by Medicaid expansion. The statistical models and results from this study can offer valuable guidance to policymakers and healthcare leaders as they formulate more efficient and effective policies.

In-hospital mortality after prehospital endotracheal intubation versus alternative methods of airway management in trauma patients. A cohort study from the TraumaRegister DGU®.

Prehospital airway management in trauma is a key component of care and is associated with particular risks. Endotracheal intubation (ETI) is the gold standard, while extraglottic airway devices (EGAs) are recommended alternatives. There is limited evidence comparing their effectiveness. In this retrospective analysis from the TraumaRegister DGU®, we compared ETI with EGA in prehospital airway management regarding in-hospital mortality in patients with trauma. We included cases only from German hospitals with a minimum Abbreviated Injury Scale score ≥ 2 and age ≥ 16 years. All patients without prehospital airway protection were excluded. We performed a multivariate logistic regression to adjust with the outcome measure of hospital mortality. We included n = 10,408 cases of whom 92.5% received ETI and 7.5% EGA. The mean injury severity score was higher in the ETI group (28.8 ± 14.2) than in the EGA group (26.3 ± 14.2), and in-hospital mortality was comparable: ETI 33.0%; EGA 30.7% (27.5 to 33.9). After conducting logistic regression, the odds ratio for mortality in the ETI group was 1.091 (0.87 to 1.37). The standardized mortality ratio was 1.04 (1.01 to 1.07) in the ETI group and 1.1 (1.02 to 1.26) in the EGA group. There was no significant difference in mortality rates between the use of ETI or EGA, or the ratio of expected versus observed mortality when using ETI.
